First order is prepaid by ACH, check or card. Net 30 is available after two orders and an approved credit application. Do not enter bank account or card numbers on this form.

This application does not create a dealer relationship. Approval is at Porceliana's discretion, and approved accounts follow Porceliana's trade terms.
Trade pricing and discount tiers are confidential and are not shared with customers or third parties.
Online and printed prices will not go below Porceliana's minimum advertised price of 90% of retail.
Products are installed per Porceliana's instructions and local codes. Power for motorized products is supplied by a licensed electrician where code requires.
Porceliana names, logos and product images are used only as Porceliana permits.
The applicant will notify Porceliana in writing within 30 days of any change in ownership, address, license or tax status.
